At Moonrise, our supported living services offer safe, flexible care that empowers individuals to live more independently in their own homes or shared accommodation.

Whether you're transitioning from residential care, moving on from family support, or simply want to live with greater autonomy, we're here to help you thrive.

Our aim is to enable people with diverse needs—including learning disabilities, autism, mental health conditions, or physical challenges—to enjoy fulfilling lives within the community, supported by a familiar and consistent care team.

Emotional Support & Respite

One-to-one engagement to reduce isolation and build confidence. We also offer respite support for family or informal carers

Our team has experience supporting individuals with Autism, ADHD, Epilepsy, Learning Disabilities, and other complex needs

Life Planning

We co-create support plans that align with the individual’s long-term goals—whether that’s employment, education, or greater independence
